Job Description
A Community Health Worker provides outreach and engagement, performs basic health screenings and social needs assessments, delivers health education/coaching, navigates patients to services, documents encounters, and collaborates with clinics, public health, and community organizations.
Qualifications (choose one)
- Graduate of a CHW, public health, or allied health training program; or
- Trained on the job by an accredited healthcare or public health organization; or
- Experience: 3 months verifiable CHW or community outreach work.
All candidates must meet age, ID, and conduct requirements per the NHWCA Candidate Handbook.
